GovForms converts your PDF and Word documents into online forms (or lets you build them from scratch), collects submissions with your chosen security model, and routes your data through the flows you configure.

Every account starts with a 14 day free trial — no credit card required. Paid plans are priced per form with unlimited submissions, billed monthly, annually (10% discount), over 3 years (20% discount) or over 5 years (30% discount).

You can cancel anytime. There are no lock in contracts, no hidden fees, no setup fees and no cancellation fees. A 30 day money back guarantee applies to new subscriptions. You may mothball your account and return to it at any time.

You retain ownership of your documents, forms and submission data. You are responsible for ensuring you have the right to collect the data your forms request and for complying with laws that apply to that data.

Where payment collection is enabled, transactions we process on your behalf incur a fee of 2.9% + $0.30 per transaction. If you connect your own Stripe or PayPal account, a fixed monthly fee applies instead.

You must not use GovForms to collect data unlawfully, to impersonate others, or to distribute malicious content. We may suspend forms that breach these terms.
